Who we are

Eli Health (operated by Lifetime Redesign OT, PLLC) is an approved participant in the CMS GUIDE (Guiding an Improved Dementia Experience) Model and is the HIPAA-covered entity that delivers clinical services. This page (caregiverbreak.com) is operated by a marketing partner under a written services agreement with Eli Health and is the front-door for eligibility inquiries.

What we collect

When you complete the eligibility form, we collect the loved one’s first name, the state they live in, your relationship to them, information about their dementia status, Medicare coverage, and care setting, and your name, email address, phone number, and preferred time to call. We also automatically collect your IP address, browser information, and the ad campaign that brought you here (if any) for security and attribution.

We do notask for the Medicare Beneficiary Identifier, Social Security Number, or date of birth on this form. Those are collected later by Eli Health’s clinical team during the formal enrollment process, if you choose to proceed.

How we use your information

• To determine whether your loved one may be eligible for the CMS GUIDE Model.
• To contact you about eligibility, the program, and next steps.
• To route qualified inquiries to Eli Health’s clinical team for follow-up.
• To measure the effectiveness of our outreach (aggregated and de-identified where possible).

We do not sell your information. We do not share it with unrelated marketing partners.

How we protect your information

Submissions are sent over HTTPS and stored in systems subject to Eli Health’s HIPAA safeguards. Access is limited to personnel who need it to contact you about your inquiry. We do not embed advertising trackers (Meta Pixel, Google Ads tags) on pages that collect or display protected health information.

Your rights

You may request a copy of the information we have about you, ask us to correct it, or ask us to delete it (subject to any legal retention obligations). Contact privacy@elihealth.com.

About the CMS GUIDE Model

The CMS GUIDE Model is a voluntary alternative payment model of the Centers for Medicare & Medicaid Services Innovation Center. Final eligibility and enrollment decisions are made by CMS. This page is not endorsed by CMS beyond Eli Health’s status as an approved GUIDE participant.
